Become part of the innovative healthcare team at Humber River Health as a Registered Practical Nurse specializing in the Hepar Dialysis and Nephrology Program. This part-time role focuses on patient-centered care and chronic disease management.
As a vital member of the nephrology team, you will empower patients to take charge of their health while integrating care throughout their kidney journey. Your skills in nursing and patient education will help optimize outcomes and patient experiences, while upholding high standards of care within our advanced digital hospital.
Key Responsibilities:
• Administer care and support for hemodialysis patients
• Educate patients on treatment and health management
• Engage in quality assurance and safety initiatives
• Cover assigned duties in Transition Care Unit
• Undertake other department-specific RPN roles
Requirements:
• Graduate RPN program recognized by the College of Nurses
• Active RPN registration in Ontario
• At least one year in acute care
• Preferred background in hemodialysis
• Competence in critical nursing procedures
