Job Summary:

MATCH is a specialized team of providers from LOFT and Inner-City Health Associates (ICHA), that delivers services to the most vulnerable individuals to assist in accessing and maintaining housing and connecting to health and other supports in the community. MATCH provides clinical hands‑on support to those with complex healthcare needs through case management and peer support services with access to psychiatric and medical assessment, consultation, access to treatment, service planning and advocacy.

MATCH works with individuals who have previously and/or are currently experiencing homelessness, whose health and needs are stabilized while working with the CATCH and M‑DOT teams. MATCH provides long‑term multi‑disciplinary wrap‑around and case management and health services in their (often new) communities. The aim of MATCH is to improve transitions of care for individuals and work with them to identify and meet their goals toward recovery, while gaining stability and building community.

MATCH
- At Home Case Manager serves individuals with serious and persistent mental illness, health and addictions issues who are homeless or recently housed. The position provides hands‑on case management services to assist individuals to make the changes in their lives that they desire in order to increase their quality of life. The position works within a Recovery and Housing First philosophy, to assess needs, develop a housing and service plan, secure desired housing, teach life skills, access community resources and supports, link to appropriate services and develop a crisis prevention/intervention plan.

The position provides access to a range of team members including a psychiatrist who will work cooperatively to assess, plan, treat and support clients.

What You Will Do:
  • Develop a plan of support and recovery which includes identifying priorities and goals, in conjunction with client, other professionals, areas identified through initial intake and OCAN’s.
  • Works in partnership with the CATCH and M‑DOT programs which provide the majority of referrals to the MATCH Team.
  • Supports housing placement through the development of life skills (i.e. shopping, budgeting, household management, use of public transit etc.,) and/or resources.
  • Establish and co‑ordinate a support network of social, family, medical, psychiatric and other community links. Develop a coordinated care plan as appropriate.
  • Develops, implements and monitors crisis prevention/intervention plans in partnership with clients.
  • Promote the services of the program and the image of the agency as a representative with clients, families, other supports, external agencies and others in a professional manner that reflects favorably on the organization.
  • Mediate issues or conflicts between residents, or residents and neighbours, teaching conflict resolution; following‑up on issues to ensure resolution.
  • Participate in planning, organizing, promoting and conducting events for the clients to address their social needs and prevent isolation; maintain a general awareness of community events and activities to also meet these needs.
  • Participate in the administration of the program by completing and maintaining the required records, assessments or statistics such as client files in accordance with established procedures.
