Support vulnerable clients as a Case Manager with The Salvation Army. This full-time role focuses on intensive case management and housing stability within a compassionate environment.
As a Case Manager, you will work closely with the Case Manager Supervisor to provide client-centered support at New Hope Norfinch. Your caseload will involve 18-25 clients, focusing on housing, needs assessment, and long-term stability. Through effective coaching and advocacy, you'll help clients navigate community resources and overcome barriers.
Key Responsibilities:
• Deliver compassionate, client-centered care to clients
• Manage 18-25 clients, providing intensive case support
• Complete assessments and tailor housing plans for clients
• Engage in client review meetings and advocate on their behalf
• Maintain accurate records and prepare required reports
Requirements:
• 2-year diploma in Social Services field
• 5+ years supporting homeless individuals
• 3+ years as a case manager
• Knowledge of community resources and housing laws
• Excellent communication and collaborative skills
