Position: Case Manager at Jean Tweed Centre

This Case Manager-Justice Access role centers on assisting adult women at the innovative Downtown East Justice Centre. With a focus on substance use, mental health, and trauma, you will create individualized service plans, coordinate with community partners, and deliver wrap-around support. Engage in transformative work that tackles the root causes of crime and enhances community well-being.

Key Responsibilities:

• Develop comprehensive service plans tailored for each participant

• Collaborate with community organizations for effective service coordination

• Provide intensive case-management support, including counselling

• Ensure linkages to both immediate and long-term support services

• Document client interactions and prepare necessary reports

Requirements:

• Bachelor’s Degree in Social Work or related field

• Minimum 3 years’ experience in related fields

• Expertise in counselling for substance use and criminal justice

• Knowledge of trauma-informed and women-centred practices

• Crisis intervention skills with effective problem-solving abilities
