Join Sherbourne as a Case Manager (Youth Resource Worker) dedicated to supporting 2
SLGBTQ youth. Play a pivotal role in facilitating care and achieving wellness goals in a full time capacity.
In this position, you will engage with young adults to assess needs and develop SMART plans tailored to their unique challenges. Your focus will be on team-oriented care coordination, crisis intervention, and outreach for group workshops. A robust foundation in social work and familiarity with community resources will enhance your impact.
Key Responsibilities- Perform intake assessments and develop support plans
- Coordinate care within an interdisciplinary team
- Evaluate outcomes and facilitate transition processes
- Lead workshops and outreach initiatives
- Keep accurate records in compliance with guidelines
- Degree in social work or equivalent training
- At least 3 years working with marginalized youth
- Case Management Certificate is preferred
- Excellent problem-solving and communication skills
- Experience in trauma-informed practices
