As a full time Case Manager (Youth Resource Worker) at Sherbourne, you will leverage your social work expertise to assess the needs of 2
SLGBTQ youth. This role emphasizes care coordination, crisis intervention, and conducting workshops to empower young adults. Your experience with community resources and a strong foundation in social work will enhance the support provided.
Key Responsibilities:
• Perform intake assessments and create individualized support plans
• Coordinate care collaboratively within an interdisciplinary team
• Evaluate outcomes and facilitate transitions for youth
• Lead workshops and outreach programs for community engagement
• Maintain accurate records following compliance guidelines
Requirements:
• Degree in social work or equivalent field
• Minimum 3 years of experience with marginalized youth
• Case Management Certificate desired
• Strong problem-solving and communication skills
• Familiarity with trauma-informed practices
