Transitional Housing Worker at CMHA

As part of the Rosemount Integrated Support Program, you will help clients navigate housing instability while enhancing their daily living skills and personal wellness. Collaborate closely with case managers and clinicians to ensure comprehensive care and to assist clients in achieving long-term housing stability.

Key Responsibilities:

• Support clients with personal care and daily living activities

• Implement case management and life skills coaching for successful transitions

• Encourage social inclusion and community building among residents

• Maintain professional boundaries while building trusting relationships

• Document client progress and coordinate care with community partners

Requirements:

• Degree/Diploma in Social Services or a related discipline

• Experience in providing direct service to adults with mental illness

• Understanding of the shelter system and outreach initiatives

• Strong interpersonal and communication skills

• Proficient in managing complex situations with sound judgment
