Youth Diversion Case Manager

Position: Contract Youth Diversion Case Manager
Join Covenant House as a Contract Diversion Case Manager, dedicated to youth facing homelessness in Toronto.

Your role will focus on collaborative solutions to prevent shelter stays.

This full-time, onsite contract position empowers you to work directly with youth aged 16-24, helping them explore safe housing alternatives. You will employ a client-centered, trauma-informed approach while engaging families and community resources to support each youth's unique needs. Your expertise will aid in securing stable housing options and providing essential case management.

Key Responsibilities:

• Engage with youth to develop individualized housing solutions

• Facilitate family and peer support systems to prevent shelter needs

• Mediate with housing providers and landlords effectively

• Implement follow-up case management for sustained housing

• Allocate emergency funding to address housing crises

Requirements:

• BSW or equivalent required; MSW is preferred

• Preferred Registration as a Social Worker

• 3+ years of experience supporting at-risk youth

• Strong collaborative problem-solving skills

• Excellent written and verbal communication abilities
