As part of your role, you will engage in direct consultations and manage casework for residents. You will conduct thorough intake interviews to identify needs and work collaboratively to create actionable case plans that lead to housing solutions. This essential position requires a focus on resourcefulness and community connections to provide ongoing support.
Key Responsibilities:
• Conduct intake interviews for ongoing resident assessments
• Provide tailored case management and consultations
• Assist residents in establishing and tracking progress toward goals
• Refer clients to essential community resources
• Attend case management meetings and maintain case documentation
Requirements:
• 2 years of College education in Social Services or Psychology
• Two years of experience in Mental Health or Addictions
• Strong English communication and interpersonal skills
• First Aid/CPR certification required
• Clean criminal records check
