Role Overview

Cota’s Supportive Housing Workers play a crucial role in assisting tenants with housing maintenance, independent living skills development, and personal recovery and wellness. Their support fosters tenants' self‑sufficiency, empowerment and overall well‑being, ultimately contributing to the success and stability of the supportive housing program.

We are seeking dynamic and innovative individuals who will work enthusiastically to deliver recovery‑oriented, client and family‑centred services in order to meet our service users’ needs and support their personal goals.

Primary Responsibilities
  • Assist with the search for and acquisition of appropriate housing units as required.
  • Coordinate intake processes, including:
    • Review referral information for applicants.
    • Participate in screening interviews with applicants and referral sources.
    • Determine eligibility and confirm acceptance of new service users.
    • Coordinate move‑in arrangements and facilitate internal move‑in practices.
    • Orient new service users to the housing environment and Cota’s services.
  • Assist with the development of Tenant Support/Goal Plans:
    • Lead the creation of support/goal plans with each tenant and their support network.
    • Document plans in the service user chart.
    • Implement appropriate housing supports to help service users achieve their goals.
    • Liaise with support networks regularly.
    • Support tenants with tenancy related matters such as rent payments, landlord disputes, reducing high‑risk behaviours, coping within a communal environment and community engagement.
  • Support activities of daily living (ADLs), including:
    • Maintain medication routines and assist with appointment reminders.
    • Help establish personal hygiene routines, laundry, unit maintenance and cleaning.
    • Assist with menu planning, grocery shopping and cooking.
    • Manage behavioural needs and other ADL supports as determined by the supervisor or team.
  • Provide other individual support requirements such as community orientation, linking to case management or other services, dispute/problem resolution and informal counselling.
  • Maintain a safe, clean environment by cleaning vacant units, common areas, and assisting with pest control as required.
  • Coordinate tenant meetings to promote direct participation in planning services and activities.
  • Plan and facilitate group activities and events that foster engagement, social networking and skill acquisition.
  • Coordinate purchasing of program supplies and account for purchases per guidelines.
  • Offer outreach support and education to landlords and housing providers, including participation in dispute meetings.
  • Document and enter data into service user charts and Cota’s database system according to guidelines.
  • Participate in team meetings and provide ideas for improving services and efficiencies.
Other Responsibilities
  • Maintain current certification and/or up‑to‑date training in required areas for MH&CS Division service providers.
  • Adhere to all of Cota’s policies, applicable program guidelines and the Ontario Human Rights Code.
  • Follow Health and Safety policies and procedures to ensure the protection of workers and service users.
  • Work cooperatively, respectfully and constructively with service users, the team, management, other service providers and stakeholders.
  • Maintain confidentiality of service users, employees and Cota information.
  • Undertake other related duties as assigned.
Qualifications

Education & Experience
