At Rosalie Hall, you will utilize your experience in social services to provide crucial support to vulnerable families. Engage with young parents through various channels, conducting detailed assessments that inform their care pathways. In collaboration with case managers, you will facilitate smooth transitions into ongoing programming and provide necessary crisis intervention.
Key Responsibilities:
• Manage intake assessments with a compassionate approach
• Build trusting experiences to ease service access
• Enter comprehensive data into client database
• Connect clients to essential services and community partners
• Follow mandatory reporting protocols while maintaining relationships
Requirements:
• CYW Diploma or equivalent education
• Minimum of 2 years in a relevant social service role
• Excellent English communication skills
• Experience with vulnerable populations
• Sound understanding of systemic health barriers
