Diversion Case Manager for Youth

Position: Contract Diversion Case Manager for Youth
Lead the Shelter Diversion program as a Diversion Case Manager committed to youth homelessness prevention. This full-time contract role requires onsite engagement to support young people aged 16-24.

We are looking for a creative and dedicated individual with at least three years’ experience working with at-risk youth. In this role, you will identify safe alternatives to shelters and coordinate with families and community partners. Your follow-up case management will help ensure housing stability for those in vulnerable situations.

Key Responsibilities:

• Engage and support young individuals facing homelessness

• Mediate with families and community partners

• Provide ongoing case management for housing stability

• Distribute funds to prevent housing loss

• Offer navigation support to community resources

Requirements:

• Minimum BSW or equivalent; MSW preferred

• Registered Social Worker preferred

• At least 3 years’ experience with homeless youth

• Strong problem-solving and advocacy skills

• Knowledge of trauma-informed practices and youth homelessness
