Job Summary

As a Case Manager you will work independently or be part of a team that coordinates a support network and links clients living with mental illnesses, addictions and physical health issues in a Cross links residence to various community services. The position attempts to empower the client, thereby increasing their independence, dignity and self-respect in a safe and secure environment. Direct service and support will be provided to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs.

This position will work with HART Hub Supportive Housing, which is led by Parkdale Queen West Community Health Centre in mid‑West/West Toronto. The program provides moderate to high support housing to individuals requiring assistance with mental health and substance use needs, aligning with LOFT’s supportive housing model that offers deeply affordable units with wraparound on‑site 24/7 support including intensive case management, life enrichment, and residential support.

The program provides trauma‑informed, client‑centred care utilizing principles of eviction prevention, harm reduction, and anti‑oppression to support individuals in their recovery journeys.

What You Will Do
  • Develop a plan of support and recovery which includes identifying priorities and goals, in conjunction with client, other professionals, areas identified through initial intake and OCAN’s.
  • Promote the personal empowerment of each individual through coaching, mentoring and support, thereby enabling each to meet the daily requirements of their living environment. Directly assist with their daily living activities, particularly in the acquisition of life skills in such areas as hygiene, medication management, cooking, cleaning, shopping and budgeting, and develop strategies and establish a plan for maintaining these daily activities.
  • Establish and coordinate a support network of social, family, medical, psychiatric and other community links. Develop a coordinated care plan as appropriate.
  • Provide crisis intervention/management and assist in developing crisis plans with clients.
  • Promote the services of the program and the image of the agency as a representative with clients, families, other supports, external agencies and others in a professional manner that reflects favorably on the organization.
  • Mediate issues or conflicts between residents, or residents and neighbors, teaching conflict resolution; follow up on issues to ensure resolution.
  • Participate in planning, organizing, promoting and conducting events for the clients to address their social needs and prevent isolation; maintain awareness of community events and activities to also meet these needs.
  • Participate in the administration of the program by completing and maintaining the required records, assessments or statistics such as client files in accordance with established procedures.
  • Participate as a team member in program delivery by providing support and consultation to colleagues, identifying gaps in service and recommending solutions, participation in meetings, planning and evaluation initiatives, committees, events and activities, orienting new staff, students or volunteers, and providing other related assistance as required.
