This role engages you in comprehensive assessment and care coordination for youth aged 14-29 facing systemic barriers. You will facilitate access to services and support individual goals while addressing mental health and crisis situations. Key Responsibilities:

• Conduct intake assessments for youth and identify their goals

• Coordinate with internal and external partners for service user support

• Determine risk factors and prioritize urgent needs

• Provide brief supportive counseling when necessary

• Maintain accurate records in the electronic medical system

Requirements:

• Post-secondary education in social work or equivalent training

• Minimum 3 years’ experience with 2

SLGBTQ youth

• Case Management Certificate and current registration an asset

• Familiarity with Electronic Medical Recording systems

• Strong knowledge of harm reduction and trauma-informed approaches
