As a Case Manager, you will work closely with women participants at the Toronto Downtown East Justice Centre. This role emphasizes a client-centered, trauma-informed approach to create individualized service plans while coordinating with various health and social service providers. Your responsibilities will include outreach, court accompaniment, and short-term counseling tailored to meet the unique needs of each participant.
Key Responsibilities:
• Establish individualized, holistic service plans for participants
• Lead collaborative processes with community organizations
• Provide intensive case management and ongoing support
• Link clients with Jean Tweed's wrap-around services
• Engage with community partners and DTE JC team
Requirements:
• Bachelor's Degree in Social Work or related field
• 3 years experience in Substance Use or Criminal Justice
• Expertise in counseling and case management
• Knowledge of trauma-informed practices
• Strong crisis intervention and problem-solving skills
