Case Coordinator

Position: Case Coordinator at Interval House
Interval House seeks a Case Coordinator in Toronto, Ontario to empower women and children escaping abuse through specialized care and assistance. Your expertise in supportive counselling and case management will directly impact clients' lives.

This full-time role focuses on delivering structured case management services to women experiencing intimate partner violence. As a Case Coordinator, you will assess client needs, develop case plans, and ensure effective support throughout their recovery journey. Your background in crisis intervention and counseling will facilitate healing and independence for clients.

Key Responsibilities:

• Implement 24-hour Crisis and Information line tasks

• Conduct admissions and discharge interviews with clients

• Schedule appointments for developing case plans

• Provide individual counseling for trauma recovery

• Track client progress and evaluate case plans

Requirements:

• Minimum 2 years’ experience with women facing intimate partner violence

• Knowledge of change counseling models

• Skills in crisis intervention and outreach

• Proficiency in case management software

• First Aid/CPR certification required
