Transitional Program Worker

Position: Transitional Housing Program Worker - Full-Time
Nellie’s seeks a Transitional Housing Expansion Program Worker for a full-time, six-month maternity leave contract. Support marginalized women and children facing housing and trauma-related challenges in Toronto.

In this role, you'll provide essential case management, crisis intervention, and community outreach for women and gender-diverse individuals dealing with gender-based violence, mental health issues, and substance use. With up to three years of relevant experience preferred, you will facilitate effective resource connections and supportive services. Your focus will emphasize trauma-informed, client-centered care that empowers clients to navigate their paths to safety and stability.

Key Responsibilities:

• Provide one-on-one case management support to clients

• Develop safety and transitional support plans

• Assist clients with housing applications and financial assistance

• Conduct crisis intervention and de-escalation

• Collaborate with community agencies for resource access

Requirements:

• Diploma or degree in Social Work/Social Services

• 1-3 years experience in client-centered care

• Knowledge of harm reduction and mental health

• Strong communication skills and advocacy focus

• Ability to manage confidential information
