The Case Manager will work directly with clients facing complex challenges, including housing, mental health, and legal issues. By utilizing a trauma-informed, harm-reduction approach, you will help participants set and achieve their self-directed goals through dedicated support. Collaborating with external service providers is crucial to deliver comprehensive resources and assistance to clients.
Key Responsibilities:
• Conduct intake and assessment interviews with participants
• Coordinate referrals and advocacy for various supports
• Facilitate ongoing community support and home visits
• Monitor housing issues and liaise with landlords
• Engage in crisis intervention and active case management
Requirements:
• BA/B.S.W with 2 years’ experience in human services
• Experience with mental health and substance use
• Commitment to anti-racism and anti-oppressive practice
• Familiarity with case management and crisis intervention
• Excellent written and verbal communication skills
Support vulnerable individuals in the GTA by leveraging your expertise in case management and community resources at TNG.
