Mobile Shelter Support Program Case Manager

As an experienced Case Manager, you will deliver supportive case management tailored to adults facing various challenges such as housing instability, health issues, and legal needs. Using a trauma-informed, harm reduction approach, your support aims to empower clients to reach their personal goals effectively. Collaboration with external service providers will also be a key part of ensuring holistic care for participants.
Key Responsibilities:
• Perform detailed assessments and coordinate necessary supports
• Engage in ongoing support and follow-up with clients
• Build partnerships with external services, including healthcare providers
• Address property monitoring and landlord communications
• Participate in crisis intervention and de-escalation efforts
Requirements:
• BA/B.S.W with minimum 2 years’ experience in human services
• Direct experience with homelessness and trauma-affected clients
• Strong knowledge of crisis intervention techniques
• Proficiency with web-based data entry and documentation
• Ability to traverse the city using transit
