In this role, you will exclusively support women participants, implementing individualized service plans while collaborating with health, legal, and social services. Your work will focus on breaking the cycle of offending by addressing underlying issues in mental health and substance use through integrated care approaches. Facilitate outreach, provide court accompaniment, and help create supportive networks for participants.
Key Responsibilities:
• Create tailored service plans with each client
• Implement a wrap-around support approach
• Facilitate outreach and assessments
• Collaborate with community health services and partners
• Document client interactions and progress effectively
Requirements:
• Bachelor's Degree in Social Work or related discipline
• 3 years of relevant experience in social services
• Knowledge of women-centered and trauma-informed care
• Strong understanding of community resources in Toronto
• Proven crisis management and interpersonal skills
Make a meaningful difference in the lives of vulnerable women through dedicated case management and support.
