Behavioural Support Case Manager

Elevate your career with LOFT Community Services as a Behavioural Support Case Manager in Toronto. This onsite role focuses on improving care for older adults with responsive behaviours through tailored support plans.

As a Case Manager in Behavioural Supports Ontario, you will engage directly with older adults and care partners dealing with dementia, mental health issues, or other conditions. You'll conduct comprehensive behavioural assessments and develop evidence-based support plans, all while coordinating care with community agencies. Your responsibilities will include direct service to families, advocating for clients, and working closely with interdisciplinary teams to ensure effective implementations.

Key Responsibilities:

• Develop individualized Behavioural Support Plans for clients

• Conduct thorough behavioural assessments and maintain documentation

• Collaborate with clients, families, and multidisciplinary teams

• Offer training and ongoing support to staff and caregivers

• Deliver crisis intervention and management plans

Requirements:

• Post-secondary education in Social Services/Human Services

• Minimum 3 years’ experience with similar populations

• Understanding of mental health and substance use challenges

• Proven crisis intervention and problem-solving skills

• Valid driver’s license and access to a vehicle
