Intake Worker for Seniors at TNG

Become an essential part of The Neighbourhood Group as an Intake and Facilitation Worker dedicated to supporting seniors. Utilize your assessment skills in a full-time contract role to enhance community living.

As part of an impactful multi-service agency, you will conduct client assessments and provide key referrals. This important position in the Independent Living department emphasizes intake processes and outreach programs for the Chinese community. You will work collaboratively with community partners and support needs assessments to ensure seamless service delivery.

Key Responsibilities:

• Conduct client assessments using the interRAI CHA tool

• Schedule and follow up on intake requests promptly

• Maintain accurate records in the database system

• Facilitate outreach events and programs consistently

• Assist with staff training for new members

Requirements:

• Diploma in Social Services or equivalent experience

• Fluent in Mandarin, Cantonese, and English

• Minimum three years in senior care assessments

• Knowledge of community resources and social services

• Capability to manage diverse client needs effectively
