Job SummaryThe Case Manager works with French‑speaking individuals with serious and persistent mental illness to provide direct case management services, facilitating recovery plans, life‑skills training, resource access, safety planning, and crisis prevention.
What You Will Do Direct Service Responsibilities- Provide support and assistance to assigned clients experiencing serious and persistent mental illness through regular community contact.
- Complete comprehensive assessments of each client’s unique goals and needs using collaborative and therapeutic methods.
- Guide clients in developing a personalized Client Safety Plan and completing the Ontario Common Assessment of Need (OCAN).
- Assist clients in achieving identified goals such as acquiring shelter, food, income, , clothing; building a positive support network; accessing primary and psychiatric care; developing budgeting, medication, and symptom‑management skills; creating harm‑reduction plans; pursuing educational or vocational interests; developing coping strategies and securing safe, affordable housing.
- Develop, implement, and monitor service care plans and crisis‑prevention/intervention plans in partnership with clients.
- Teach life skills (shopping, budgeting, household management, public transit use, etc.).
- Help clients identify and manage symptoms, develop effective coping skills, and address emergency or crisis needs.
- Coordinate additional community resources, supports, services, and appointments.
- Maintain liaison with clients’ support networks when consent is obtained.
- Provide services within an anti‑racism/anti‑oppression framework.
- Advocate on behalf of clients when appropriate.
- Establish and maintain positive working relationships with other service providers, professionals, agencies, and families.
- Develop discharge plans in partnership with clients.
- Perform other duties as assigned by the Senior/Project Manager or Executive Director.
- Maintain confidentiality, privacy, and accuracy of all client and agency information in line with policies.
- Record accurate, clear, concise client case notes in the agency database.
- Organize client paper files.
- Manage privacy and accuracy of information in shared solutions such as IAR, HPG for CHRIS, and Better Care.
- Submit monthly timesheets and expenses following guidelines.
- Participate in partnership, team, supervision, administration, and peer group meetings to improve services.
- Identify areas for professional growth and training in collaboration with the Senior/Project Manager.
- Participate in agency committees.
- Provide backup support during staff absences.
- Represent the agency at external workshops, inter‑agency initiatives, staff training, and community activities.
- Adhere to all agency policies, procedures, and health and safety guidelines.

- A post‑secondary education in social services or health care and at least three years of related experience.
- Fluency in French, both oral and written; only French speaking candidates will be considered.
- Knowledge of psychosocial rehabilitation principles, including the philosophy of recovery.
- Experience with concurrent disorders and addiction services.
- Knowledge of community resources for mental health and substance use.
- Excellent interpersonal skills and supportive counselling techniques, especially with marginalized populations.
