As a Case Manager at HART Hub Supportive Housing, you will work directly with clients, developing recovery plans, and creating a secure support network. • Design recovery and support strategies with clients
• Directly assist in daily living activities
• Foster a comprehensive support network
• Provide crisis intervention and develop response plans
• Maintain accurate client records and statistics

• Relevant post-secondary education in human services
• Minimum of three years experience in related fields
• Solid communication and counseling abilities
• Valid driver’s license and reliable transportation
• Knowledge of community services and supports
