Position: Housing Transition Coordinator in Northeast Ohio
Support adults in achieving housing independence as a Housing Transition Coordinator in Northeast Ohio. This role focuses on community outreach, assisting clients in transition from long-term care settings.

In this role, you will serve as a critical link between healthcare providers and housing resources. As an experienced Community Health Worker or STNA, you will utilize your knowledge of community services to help individuals secure housing and entitlement benefits. Your work will be essential in facilitating a smoother transition for clients needing support.

Key Responsibilities:

• Liaise between healthcare and housing professionals

• Assist clients in locating suitable housing

• Coordinate required services and resources effectively

• Help clients access benefits and necessary supplies

• Provide exceptional customer service and support

Requirements:

• Minimum 1 year in social services or healthcare

• High school diploma or GED required

• Knowledge of community resources and housing services

• Valid driver's license for client travel
