Case Manager for Mobile Shelter Support

Become a vital part of TNG's Mobile Shelter Support Program as a Case Manager. You'll deliver direct case management and support to vulnerable adults in shelters and encampments across the Greater Toronto Area.

This full-time position in Urban Health and Homelessness Services involves working with individuals facing health, housing, mental health, and legal challenges. You will provide case management through in-person and telephonic support, helping clients set and meet their own goals while ensuring they connect with essential services. Your contributions will significantly impact the lives of those experiencing homelessness in the community.

Key Responsibilities:

• Conduct intake and assessment interviews with participants

• Coordinate and advocate for health, financial, and housing support

• Help participants achieve self-directed goals

• Maintain ongoing community support through visits and appointments

• Develop partnerships with external service providers

Requirements:

• BA/B.S.W and 2 years of experience in human services

• Direct client experience with mental health and substance use

• Commitment to harm reduction and anti-oppressive practices

• Strong crisis intervention and conflict resolution skills

• Proficient in MS Office and data entry for case management
