In this position, you will provide essential triage and case management support for individuals with developmental disabilities within the Toronto community. You will be instrumental in managing referrals and connecting clients with vital resources. Your expertise in trauma-informed care will help guide your work, ensuring clients and their families receive the tailored support they need.
Key Responsibilities:
• Triage and assess client referrals for case management support
• Maintain and manage the case management referral waitlist
• Input client data into electronic records accurately
• Refer clients to appropriate internal and external resources
• Advocate for clients with families and service partners
Requirements:
• Post-secondary degree in social service work or equivalent
• Minimum three years of clinical/case management experience
• Knowledge of developmental services sector referral processes
• Experience developing individualized service plans
• Strong interpersonal, organizational, and communication skills
