Case Manager for CARE Centre

Join CARE Centre for Internationally Educated Nurses as a Case Manager in a hybrid role. Support IENs in Ontario, primarily in the Greater Toronto Area, by guiding their pathways to nursing licensure.

This full-time, one-year contract position requires collaboration with a dedicated team and various stakeholders. You'll leverage highly developed case management skills to assist internationally educated nurses in navigating Ontario's complex healthcare landscape. The role emphasizes client intake, support, and community outreach, making a significant impact on IENs’ professional journeys.

Key Responsibilities:

• Conduct screenings and assessments per enrollment requirements

• Guide clients through registration processes and requirements

• Develop individualized action plans for clients

• Monitor client progress and adjust plans as necessary

• Assist with job search tasks, including resume writing

Requirements:

• Degree in Nursing, Education, or Social Services

• Experience in case management within healthcare or education

• Strong understanding of Ontario's health care system

• Excellent communication and facilitation skills

• Ability to maintain an electronic client database
