Developmental Services Triage Manager

As a Case Manager, you will offer critical support to clients, managing their referrals and ensuring they receive appropriate resources. You will play a key role in advocating for clients and families while overseeing the case management process. Collaborating with service providers and community resources, your work will help navigate clients through the developmental services landscape.
Key Responsibilities:
• Assess and triage client referrals from Developmental Service Ontario
• Manage the DS case management referral system for the Toronto region
• Respond to inquiries from clients and service partners
• Support clients in identifying strengths and goals
• Monitor and implement individualized service plans
Requirements:
• Degree in social service work or related field
• Three years of clinical experience with developmental disabilities
• Experience with individualized service planning
• Strong problem-solving and communication skills
• Understanding of community resources in Toronto
