Facilitation Worker in Community Services

Join The Neighbourhood Group as an Intake and Facilitation Worker, specializing in community service for seniors. This role focuses on client assessments and resource referrals in a full-time contract position.

Working within the Independent Living department, your responsibilities include assessing clients, offering referrals, and organizing programs specifically for the Chinese community. Collaborating with community stakeholders and maintaining accurate records are essential aspects of this role, amplifying TNG’s service impact.

Key Responsibilities:

• Respond to client intake requests and schedule assessments

• Liaise with clients for smooth transitions to services

• Keep comprehensive documentation of client interactions

• Coordinate outreach events for community engagement

• Train and guide new staff and volunteers in service delivery

Requirements:

• Social Services diploma or relevant education

• Fluency in Mandarin, Cantonese, and English

• At least three years of experience in senior care

• Familiarity with social services policies and resources

• Strong advocacy skills and service coordination
