Join Baycrest as a part-time Social Worker and provide crucial support to older adults with complex needs. This role emphasizes patient advocacy and collaboration within an interprofessional healthcare environment.

As a Social Worker, you will engage in psychosocial assessments, discharge planning, and resource navigation for patients and their families. This temporary part-time role requires a Masters of Social Work and a valid registration with the Ontario College of Social Workers. You'll play a key role in connecting families to community resources while ensuring effective transitions in care for older adults.

Key Responsibilities:

• Facilitate psychosocial assessments and crisis intervention

• Support patient discharge planning and family counseling

• Act as a liaison for Ontario Health at Home

• Liaise with interdisciplinary teams regarding psychosocial issues

• Develop outreach with community services and agencies

Requirements:

• Masters of Social Work degree

• Current registration with OCSWSSW

• Preferred experience in healthcare and older adult care

• Strong communication and interpersonal skills

• Proficiency in Meditech and Microsoft applications
