Key Responsibilities:

• Encourage active participation and foster client engagement

• Provide guidance in navigating community resources and applications

• Monitor and assess client well-being and progress

• Perform delegated health-related tasks, including medication management

• Facilitate housing stabilization through collaboration

Requirements:

• Diploma in Social Service Work or related field

• Minimum of 2 years' experience supporting marginalized populations

• Ability to resolve conflicts and manage crises effectively

• Strong organizational skills and documentation abilities

• Knowledge of geriatric issues and community services in Toronto
