Indigenous Wellness Specialist

Position: Full-Time Indigenous Wellness Specialist
Enhance Indigenous health and community ties with Unity Health Toronto as a full-time Indigenous Wellness Specialist. Support families navigating primary care while leveraging culturally safe practices to promote wellness.

This role within the Unity Health Indigenous care team involves connection with the SMH Family Health Team, crucial for providing primary care to over 45,000 patients. As an Indigenous Wellness Specialist, you'll engage directly with Indigenous individuals and families, assisting in their health care journey by offering access to resources and services while liaising with foundational organizations.

Key Responsibilities:

• Conduct intake assessments for new Indigenous patients

• Assess needs using culturally safe methods

• Facilitate referrals to community resources

• Support patients during appointments and community events

• Document and maintain accurate client information

Requirements:

• Indigenous identity with local ties as asset

• Minimum 2 years experience in community-based services

• Knowledge of social services and health promotion

• Ability to engage marginalized clients respectfully

• Strong organizational and digital skills
