Position: Social Worker Needed Western Hospital
Empower patients at the Movement Disorders Clinic of Toronto Western Hospital as a Social Worker focused on providing psychosocial support and system navigation. This temporary full-time role will enhance patient access to necessary care.

As a Social Worker, you will partner with patients and healthcare providers for assessment and tailored support. Emphasizing education and coordination with various services, your work aims to address financial and logistical barriers for those affected by movement disorders. Your proactive approach will guide patients towards effective resources and support.

Key Responsibilities:

• Conduct psychosocial assessments for patient care access

• Deliver time-limited counselling and facilitate education

• Help identify barriers in accessing community services

• Collaborate on patient care planning with the clinical team

• Assist with documentation and follow-ups for consultative interventions

Requirements:

• Master's Degree in Social Work is essential

• Registration with the Ontario College of Social Workers

• Experience in complex disease or outpatient settings preferred

• Strong verbal and written communication skills

• Ability to work well within a team environment
