Position: Navigation & Resource Specialist Role
Become a Navigation & Resource Specialist at Wood Green, where you'll enhance community support and access to healthcare. This full-time position involves direct client care and resource navigation.

Reporting to the Counselling & Support Services Manager, you are tasked with conducting comprehensive assessments and creating client-centered goals. You will actively engage with individuals, families, and care providers, ensuring they have access to essential services related to mental health and substance use. The focus is on empowering clients and fostering community well-being.

Key Responsibilities:

• Provide timely client intake and assessments

• Identify credible community resources for diverse needs

• Facilitate effective client-provider communication

• Coordinate group programs on health and wellness

• Manage and document client interactions efficiently

Requirements:

• Master’s Degree in Social Work or related discipline

• 3 years of relevant experience in social services

• Valid Ontario Social Worker registration

• Proven insight into trauma-informed care

• Ability to work collaboratively within a multi-disciplinary team
