Key Responsibilities:
• Supervise a diverse team of social workers and assistants
• Oversee participant assessments and intervention plans
• Ensure compliance in care documentation and reviews
• Facilitate programming in collaboration with specialists
• Conduct home visits as required
Requirements:
• Social work licensure preferred
• One year experience with elderly populations
• Excellent organizational and communication skills
• Clean state driver’s license and up-to-date vehicle insurance
• CPR and First Aid certification needed within 180 days
