Case Manager – Carlton House (Full Time Permanent)

The Canadian Mental Health Association is looking for a Case Manager for Carlton House. Carlton House provides Transitional Shelter for Refugees and newcomers with embedded Mental Health and Addictions Supports. The Case Manager will support the residents' mental health and social care needs, ensuring access to primary care and navigation of health and social care systems. This model of care will ensure seamless and coordinated support for refugees settling in Toronto.

Reporting to the Program Manager, the Case Manager will:

• Provide on-site (213 Carlton St., Toronto) mental health and addictions support tailored to residents' needs, including developing personalized care and relapse prevention plans.
• Deliver settlement counseling services, including assessment, information, orientation, and shelter support at the Carlton House location.
• Facilitate internal/external referrals for services not provided by the program, including specialized and individualized supports.
• Organize, plan, and facilitate group and individual sessions for skill-building, life skills training (e.g., household maintenance, budgeting), and addressing diverse client needs.
• Develop and implement community outreach plans to promote programs and services.
• Collaborate with clients, programs, settlement services, and relevant parties to create individualized case management plans, including access to primary care, employment resources, and navigation through health, social care, and legal systems.
• Provide flexible support, including evenings and weekends, and assist residents with their migration journey and integration into the community.
• Offer transitional shelter support, develop safety and long-term housing plans, and create post-transitional support plans for ongoing housing and social care needs.
• Along with the Clinical Lead, develop and codesign a ‘mentorship’ program for clients in the home.

The role will involve conducting client assessment/individual service planning and develop therapeutic alliance with clients and their support systems. Further, the Case Manager will build and maintain a trusting respectful and collaborative working relationship with client and be able to provide supportive counseling for a range of issues, skills teaching as well as psycho-education around illness, medications and symptoms.

This role will involve direct service provision/intervention with a trauma informed and strength-based approach. The Case Manager will also develop, implement and monitor the service plan and conduct follow-ups with the clients as needed. The Case Manager will advocate on behalf of the client and connect them with community resources as well as establishing effective working relationships with staff of other agencies.

The candidate must be able to work flexible hours, including evenings and weekends.

CMHA Toronto’s ideal candidate will have an undergraduate degree or diploma in social services or related field. The candidate will bring a minimum of two years’ experience in direct service delivery and work experience in the settlement and/or community work sectors to adults with psychiatric disabilities.
