Onsite Case Manager

Reporting directly to the Supportive Housing Supervisor, the Case Manager provides essential services to tenants managing prolonged mental health and addiction challenges.

Your role centers on comprehensive case management, crisis intervention, and support for those navigating the complexities of homelessness. You will work closely with multidisciplinary team members to ensure holistic client wellness and sustainable housing solutions.

Key Responsibilities:

• Provide comprehensive case management and develop individual case plans

• Assist tenants with daily physical, emotional, and social support

• Facilitate connections to community resources and health care

• Monitor housing adherence and develop intervention strategies

• Complete records and maintain statistics on program enrollment

Requirements:

• Bachelor's degree in Social Work or related field

• 3+ years of experience in mental health and housing support

• Proven skills in case management and crisis intervention
