Your work will revolve around intake processes, assessing client needs, and facilitating access to resources. By implementing strategies for effective community collaboration, you’ll promote wellbeing and address critical needs within the community.

Key Responsibilities:

• Manage client referrals and intake processes

• Perform thorough clinical assessments to identify service needs

• Cultivate partnerships for improved community service delivery

• Facilitate access to resources for clients and their families

• Document and maintain accurate client statistics

Requirements:

• Postgraduate Degree, preferably in Social Work

• 3+ years of experience in related fields

• Registered with Ontario College of Social Workers

• Strong communication and documentation skills

• Experience with evidence-based therapeutic practices
