Hours/Shifts Flexibility to work varying shifts:
Monday to Sunday: 7am to 3pm, 3pm to 11pm, 8am to 4pm, 4pm to 12am. Flexibility to cover shifts is required. Schedules are subject to change to meet operational needs.
Developmental Services' TAY home is a supported living program that provides transitional aged youth (TAY) with developmental disabilities, supporting their transition from child welfare and development of life skills for independent living.
Responsibilities
Needs Analysis
Collaborate with individuals with developmental disabilities to develop goals, plans, and strategies that align with their unique needs and enhance independence.
Conduct effective needs analysis in collaboration with the individual to identify unique needs.
Service Delivery
Support the delivery of services per goal plans to foster transition to independent living.
Assist senior DSWs in locating e‑programs, services, housing, and employment opportunities aligned with goal plans.
Work collaboratively with TAY staff to develop strategies and implement therapeutic programming.
Ensure personal and physical care, crisis response, counseling, and emotional support are provided in an effective, trauma‑informed manner.
Program Development
Support development of programs within and outside the home, implement life‑skills facilitation enabling independent community integration.
Ensure program plans promote independent living, reflect high learning levels, and are developed collaboratively with the Supervisor.
Facilitate life‑skills programming individually or in groups, based on needs, goals, and support plans, within timelines and standards.
Participate in assessing long‑term service needs and program evaluation.
Community Outings
Provide supports for individuals to actively participate in community activities and recreational outings safely.
Plan outings, appointments, and activities meeting safety protocols and promoting social inclusion.
Crisis Management
Reduce safety incidents through strategies that reinforce positive behavior and identify behaviors before, during, and after incidents, collecting objective data as required.
Apply Individual's Behavior Support Plans consistently during crises or challenging behavior.
Program and Administrative Duties
Produce professional written materials and correspondence that reflect agency philosophy and privacy standards.
Monitor finances for individuals as required, following policies and practices.
Qualifications
Education / Experience
Post‑secondary diploma/degree in Social Services or Developmental Services, or a degree/diploma in a related area with at least two years of experience in developmental services.
Valid CPR/First Aid and Crisis Prevention Institute (CPI) certifications.
Strong knowledge of MS Word, Excel, PowerPoint, Outlook.
Minimum two (2) years of recent work‑related experience in the following areas:
Working directly with individuals with developmental disabilities or dual diagnosis.
Case management experience.
Supporting youth in the child welfare system.
Approach working from trauma‑informed and harm‑reduction perspectives.
Engagement with diverse populations, applying a culturally responsive lens in all interactions.
Understanding of the Social Inclusion Act and the Act for the Supports of Persons with Developmental Disabilities (2008).
Working knowledge of the Developmental Services Sector.
Skills / Abilities
Knowledge of the Developmental Services Sector.
Proven individual and group facilitation skills leading to increased independence.
Assessment skills for service delivery and program operational issues.
Program planning skills for implementation, evaluation, and modification.
Understanding of Trauma‑Informed Care practices.
Effective crisis intervention skills and ability to work well under pressure.
Written communication skills in English.
Verbal communication skills in English.
Nice to Have
Ability to communicate in French verbally and in written form.
