Position: Social Worker - Primary Care Team Role
Elevate your social work expertise as a part-time Social Worker with the Scarborough Health Network's Interprofessional Primary Care Team. This role allows you to directly impact patient care through assessment and intervention strategies on weekdays and weekends.

As a qualified Social Worker, you will leverage your MSW degree to assess social determinants of health and plan interventions that mitigate barriers to care. Your collaboration with various healthcare professionals will ensure effective delivery of client-centered services, enhancing support for patients and families in the community.

Key Responsibilities:

• Assess and address patient barriers to recovery

• Collaborate with healthcare team for quality care

• Promote patient inclusion in care plans

• Document patient encounters and maintain communication

• Provide mentorship and leadership to new social work students

Requirements:

• Master of Social Work (MSW) degree is essential

• Registered with the Ontario College of Social Workers

• Experience in community or primary-care environments

• Strong focus on psychosocial assessments

• Excellent written and verbal communication skills
