Client Support Worker

Join Wood Green as a Client Support Worker and support seniors overcoming homelessness in Toronto.

Your role will include crisis intervention and promoting community engagement through comprehensive support.

This position is permanent full-time and requires working onsite at 841 Queen St E, Toronto. You will provide emotional support, assist with care planning, and encourage client participation in various activities. Utilize your social service diploma and experience working with vulnerable populations to enhance the well-being of clients.

Key Responsibilities:

• Build supportive relationships with program participants

• Provide one-on-one assistance with care plans

• Participate in client intake and onboarding processes

• Monitor client well-being and adapt care plans

• Support goal setting for community integration

Requirements:

• Social Service Worker diploma or related education

• Minimum 2 years' experience in social services

• Skills in crisis intervention and de-escalation

• Knowledge of community resources in Toronto

• Ability to lift up to 40 lbs
