Key Responsibilities:

• Conduct psychosocial assessments and crisis interventions

• Support team members with complex patient situations

• Lead discharge planning and patient flow initiatives

Requirements:

• Master's degree in Social Work

• Valid OCSWSSW registration

• Experience in discharge planning preferred

• Background in healthcare settings preferred
