Strides Toronto Support Services ("Strides Toronto") Strides Toronto is a multi-service agency supporting infants, children, youth and young adults from birth to age 29, and their families, primarily across east Toronto. We offer community-based, residential and treatment programs—including mental health counselling, autism services, early intervention, day treatment, education, outreach and community support—using a client-centered, evidence-informed approach. Since 2015, Strides Toronto has served as Toronto's Lead Agency for Infant, Child and Youth Mental Health, partnering with 21 core service providers to provide system leadership, citywide planning, coordinated service delivery and program alignment.

Strides Toronto also leads initiatives with broader reach, including Help Ahead and the HOPE Strategy across Toronto and One Stop Talk / Parlons Main tenant and the Provincial Youth Outreach Worker Program across Ontario.

This is an existing full-time, permanent Urban Tele-Mental Health Coordinator position responsible for facilitating coordinated access to virtual psychiatry services through The Hospital for Sick Children (Sick Kids). The coordinator provides centralized coordination and liaison support for community partners and the Sick Kids Tele-Mental Health Program. The role also leads community engagement, outreach, and promotion of Child and Youth Urban Tele-Mental Health Services, fostering collaborative partnerships with community agencies and healthcare providers across Toronto and the Greater Toronto Area (GTA) to improve access to specialized mental health care.

• Support case workers with community referrals and liaise with Sick Kids to book and confirm virtual psychiatric consultation appointments.
• Record all referrals in the internal database and document the status of the referral throughout the process.
• Increase awareness and usage of the services through community outreach initiatives.
• Develop and maintain an outreach plan to ensure that community agencies are engaged and informed of the services.
• Serve as the representative for urban site perspectives within the Tele-Mental Health Network, ensuring their experiences inform program planning and service delivery.
• Identify opportunities to develop new community partnerships while fostering, managing, and maintaining strong relationships with existing partners.
• Ensure documentation is recorded accurately and reflected in a quarterly and annual summary report for the Ministry of Health.
• Provide administrative and operational support to the team, including coordinating meetings, maintaining documentation and supporting day-to-day program activities.

The following education, professional memberships and knowledge are a requirement for this role:

• Post-Secondary Education in Human Services or a related field.
• Minimum of 2 years of administrative experience.
• Sound knowledge of databases.
• Intermediate computer proficiency in order to use Microsoft Office, Outlook and other computerized systems and databases.
• Knowledge of CYMH Services/Hospital Services.
• Knowledge of UTMH Services through Sick Kids.

While not required, preference will be given to candidates with the following:

• Trained in Conflict Resolution, Crisis Prevention and Intervention and CPR would be considered an asset.
• Knowledge of specialized agencies/services that may access services is an asset.
• Fluency in another language(s), including sign language.
