Program Overview – Cummer Housing Program, Supportive Housing – Community Care Unit

Reporting to the Supportive Housing Supervisor, the Case Manager provides specialized support to tenants within our Supportive Housing Programs who are living in bachelor-style units. These individuals often navigate prolonged mental health and addiction challenges or have experienced chronic homelessness. This position is vital to maintaining affordable and sustainable housing, with a primary focus on providing specialized interventions for hoarding and pest control management.

Working as part of a multidisciplinary team of Client Support Workers and Case Managers, you will provide comprehensive clinical support, including client assessments, case planning, and crisis intervention. You will act as a key liaison between our clients and the broader community by making referrals to appropriate resources and collaborating closely with healthcare providers and agency partners to ensure a holistic approach to client wellness.

What You Will Do

Provide comprehensive case management services: assessment, personal goal setting through the development of Case Plans, short/long-term planning, informal counselling, follow up support, crisis intervention, service co-ordination, case conferencing, and referrals.
Undertake needs assessments and develop case plans with individuals in the program.
Assist tenants with mental health issues by providing a variety of day-today physical, emotional, and social supports, life skills assistance, information, and resources.
Identify and address mental health issues connecting them with primary care and other needed resources and support them to utilize informal and formal support networks appropriately.
Work closely with staff and clients on strategies that support clients with substance use and mental health issues to maintain their housing and advocate on behalf of clients when required.
When required, accompany clients to offsite medical appointments, and any other community-based services and provide emotional and logistical support to clients during offsite visits, ensuring they understand appointment details, advocate for their needs, and stay engaged throughout the process.
Link clients to various community resources and facilitate appropriate referrals to other community support services when required.
Ensure the coordination of services and a “continuum of care”.
Liaise, consult, and collaborate with other program area staff and service providers.
Monitor the housing challenges and ability of each resident to adhere to the policies and protocols and develop a team strategy for intervention when problems occur in accordance with housing first and eviction prevention principles.
Complete statistics/reports and maintain all records regarding program/activity enrollment.
Ensure that the anti-racism objectives and community development principles of the organization are achieved in the program.
Gather statistical and narrative information required for the Service and organization.
Might be exposed to second-hand smoke, behavioral & cognitive outbursts, death, dying, drug use, pest-related concerns etc.

What You Bring to the Team

Bachelor’s degree in Social Work or a related discipline.
3+ years related experience in mental health and harm reduction, housing help, and working with those who have experienced homelessness.
