Wood Green Community Services in Toronto is seeking a Case Manager for the Cummer Housing Program. This full-time, bargaining-unit role offers 35 hours per week, Monday to Friday, onsite at 185 Cummer Avenue.
You will provide specialised case management, assessments, planning, crisis intervention, and referrals to support tenants facing mental health, homelessness, and housing stability issues, with emphasis on hoarding and pest control management.
