Holland Bloorview Kids Rehabilitation Hospital in Toronto seeks a Social Worker to support children and youth with disabilities and their families, including newcomers. You will provide upfront supports while awaiting their first appointment, and work both independently and with outpatient teams to address unmet social needs.
You will collaborate with an interdisciplinary team, advocate for client/family needs, and integrate equity-oriented care practices as part of daily work.
