Social Worker for Long-Term Care

At Yee Hong, we are opening our newest long-term care home and searching for passionate Social Workers. You will provide counseling and social support to residents and their families, contributing to a nurturing environment. Your experience in counseling and group work, along with empathy and excellent communication skills, will be pivotal in making a positive impact.

Key Responsibilities:

• Conduct intake assessments and provide service coordination

• Offer supportive counselling to clients and families

• Collaborate with interdisciplinary team on care plans

• Provide crisis intervention and short-term assistance

• Develop social work service policies in consultation

Requirements:

• Bachelor’s degree in Social Work and registration in Ontario

• Minimum 2 years of counseling experience

• Proficiency in computer applications

• Ability to converse in Chinese

• Satisfactory Police Vulnerable Sector Screen result
