In this temporary part-time position, you will engage with older adults and their families, developing care strategies that emphasize emotional support and successful healthcare transitions. Collaborate with a dedicated interprofessional team while enhancing patient outcomes through effective discharge planning and counseling.

Key Responsibilities:

• Conduct thorough psychosocial assessments and crisis interventions

• Facilitate discharge planning and patient education

• Consult with multidisciplinary team members on patient issues

• Act as liaison with Ontario Health at Home services

• Develop community connections with relevant agencies

Requirements:

• Master’s degree in Social Work required

• Valid registration with Ontario College of Social Workers

• Preferred experience in healthcare and with older adults

• Strong problem-solving and communication abilities

• Familiarity with Meditech and MS Office applications
