Key Responsibilities:

• Conduct assessments and oversee service coordination

• Provide individualized and group counseling

• Collaborate to implement resident care plans

• Deliver crisis intervention and short-term solutions

• Contribute to policy development on social work services

Requirements:

• Bachelor’s degree in Social Work and relevant registration

• At least 2 years of experience in long-term care

• Proficiency in relevant software applications

• Empathetic approach to resident care

• Valid Police Vulnerable Sector Screen
